Coming in for Your First Visit?
Your initial appointment will consist of a very thorough oral examination that includes hard and soft tissue analysis, temporo-mandibular joint (TMJ) exam, occlusal analysis and a complimentary aesthetic assessment.
To save time, we recommend that you download and print the forms located to the left and complete them prior to your visit. If you do not wish to have x-rays sent from a previous dental office, you do not have to complete the x-ray release form.
If you require treatment beyond cleaning, we will custom-design a plan for you, which our treatment coordinator will explain in detail. We will also provide all this information in writing, including treatment fees so there are no surprises in the end – except perhaps the pleasant surprise of seeing your beautifully restored smile for the first time.
